These are the links I found and what I had planned to use them for...

-You should be able to see the various activities offered, 'canciones de hip hop' are songs with lyrics written to include vocabulary and grammar.  There are also practice activities, audio files, exam review, and puzzles.

-I planned to use this to find student interest [I did not create a specific survey.]

-This is used to make story books online.  There is a free version and a version you pay for.

I too use surveymonkey quite often as a way of student self assessment and a way to anonymously gather information about what students believe is occurring within the classroom. I have found it particularly beneficial after group activities, multiple step projects, and Socratic seminars.
