Qt5 support

Hi tea,
Mac, Windows, and Linux support for Qt5 is now widespread. I would
like to migrate to Qt5 and you already wrote some patches to do that.

Do you want to rebase your patches and clean them up?

Don't worry about Qt4 compatibility. Binaries/packages of Qt5 are
available across all platforms so Qt4 support is not necessary (it
would just be extra code paths to test).

If you are busy, let me know and I can work from your patches.
