What steps will reproduce the problem?
1. Precise steps are unknown. Appears to occur most frequently when the
same file is edited in 2 consecutive revisions, but not consistently or
exclusively.
2. Has occurred with growing regularity while loading a 5GB dump file.
3. Splitting dump file and simply rerunning second half appears to resolve
issue and runs to next failure point (if any).

What is the expected output? What do you see instead?
