Asteirsk and Channels getting mixed

Has anyone ever had to deal with secondary conversations getting mixed
into a new or existing conversation while using Asterisk PBX? I have a
use who states that he initiated a call by picking up the phone,
dialing the number and when it was connected he could clearly hear two
other users on the same system having a conversation. He indicated
that this happened with several successive attempts. I have not been
able to duplicate this problem nor do I know where to begin to try and
track down this type of problem. It was my impression that this type
of thing could not happen on a VOIP system but there must be some
exceptions to that rule. Any help would be appreciated.
