Application device and OS device should be equal!
You can edit Application device and OS device by double clicking them.

Additional options is the GUID of corresponding device options object (GUID OF OBJECT in Object tab)

Using Visual BCD editor you can create a completely new loader and a new device element object and then edit every element as needed (also create new elements if needed).
Your recovery (wim) loader and device options object should look similar as posted images above (only device settings should be different and eventually sdi path).
You can also manipulate/create/edit recovery loaders using "ReAgentC" system utility (in \Windows\System32\). See docs.
