the SECN firmware will do this. With regards to practical experience I haven't tested it myself with VOIP WiFi phones.

I can connect via multiple SECN type Ap setups quite fine, connecting
to my asterisk. Phone infrastrucure mode, I have duplicated the ssid's
on the nodes so phone just picks up the strongest sig.
I havent tested while actually roaming though.
Still havent got a connection via the Ad-hoc though , would have to be
using batman for the routing.

There has been some testing done around laptops and smart phones connected to a multinode secn mesh just as wifi clients and roaming between nodes that are dispersed physically.
The roaming behaviour depends to an extent on the particular wireless client and its software but modern devices seem to handle it well, to the extent that a voip call can be carried on while roaming between nodes. Helge Reikeras did work on this in May last year.
The mesh interface in secn is intended to be used only for the network backbone and not for client devices to connect to. Client devices only connect via wifi or ethernet.
If you want to connect client devices to the mesh then the Serval project may be useful for you.
