Dear Sir,

I hope this message finds you well.

My aunt, who is residing in Canada, is currently receiving the family pension through the SPARSH platform. I have reviewed the recent instructions regarding the submission of life certificates, which indicate that we can now submit the certificate via email to sparsh...@gov.in.

I have a few queries for which I seek your assistance:

1. Is it necessary to register an email address on the SPARSH portal before submitting the life certificate, or can any email address be used for this purpose?

2. The instructions mention that for the submission of a life certificate, the subject line should read: "Annual Identification – ColXXXX – SPARSH PPO No 101XXXX." Could you kindly clarify what the subject line should be for family pensioners, given that my aunt is receiving a family pension after the demise of her spouse?

3. Where can we download a blank Form 5 for the life certificate submission?

My mother lives in USA and is a defence pensioner. Her SPARSH account is active. We use VPN to access SPARSH from USA. We use her Indian relative’s mobile number for SPARSH.

This week we got her life certificate from a nearby State Bank of India branch and submitted it on SPARSH portal. The portal generates a blank MLC form with a unique serial number (MLC Form Number) but we did not use that form. Instead we used the certificate provided by SBI on it’s letterhead and uploaded it to SPARSH portal. After uploading, the portal asks for details like name and designation of the person signing the certificate, his phone number, bank address and email ID. We provided all the details as requested, including the MLC Form number.

After uploading we first got SMS “You have successfully submitted your Life Certificate…”

After 2 days we got another SMS “Your identification process has been approved..”

On SPARSH portal the next Annual Identification date is showing as 31 October, 2025.
