Dear Indian Military Veterans and Veer Naaris,

After I sent you the last email – Email 102/2018, on 05 Aug 2018, a new development took place at the HQ ECHS.
Please see the letter issued by HQ ECHS on 03 Aug 2018, appended below and also attached to this email.
2.
In my opinion most of the Pre-1996 veterans and families, who don’t have the copy of MRO, will have only option -
to submit an Affidavit.
3. To help the Pre-1996 retiree veterans and families a draft Affidavit is appended below and also attached in MS WORD format.
4. The Affidavit for having lost/misplaced the copy of MRO, vide which you applied for ECHS membership, is to be prepared on a Rs 10.00 or
₹ 20.00Non Judicial Stamp Paper and signed by you in presence of Registered Notary and counter-signed by the Notary.
Carry your ID proof when you go to the Notary.

A blank copy of the Contingent Bill you need to submit, is appended below and also attached in MS WORD format.
You need three copies the contingent bill. One copy you will sign after affixing the revenue stamp.
One copy of the contingent bill, after duly stamped and signed by the clerk receiving all the documents, will be returned to you for your record.

Please ignore the language used in the Affidavit and Contingent Bill – a typical “Babu Language” being used since centuries.
Type it as it is.
7. To avoid you referring to the previous email on the subject, dated 05 Aug 2018, all the documents are re-attached to this email and also reproduced below.

GUIDELINES FOR PRE-1996 RETIREES/ FAMILIES
FOR CLAIMING REFUND OF Rs 18,000.00 PAID FOR THE ECHS ENROLLMENT
Items required for claiming refund of Rs 18,000.00
(or less amount) paid by
Pre-1996 retiree ESM / Families for registering their and their families names for ECHS: -
1. Copy of MRO vide which amount was paid, or the alternatives as mentioned in the letter of HQ ECHS, dated 03 Aug 2018.
2.
Photocopy of ECHS Card, self attested
(card only of the claimant required and not of dependents).
3. Photocopy of PPO self attested.
4.
“Mandate Form” duly completed and countersigned by the Branch Manager of your Bank, where the refund will be received.
It is suggested carry your photo identity to the Bank.
5. A cancelled cheque slip of the concerned bank.
6.
Contingent Bill 3 copies to be signed by the ESM/ spouse.
On one copy affixed the Revenue stamp before signing.
The third copy of the Contingent Bill will be returned to you, after duly receipted the receiving clerk.
7.
The Contingent Bill and other documents will be forwarded by the concerned ECHS Regional Centre/ Receiving Office to the Regional CDA.
After due processing by the concerned CDA, the amount will be credited to the bank account of the ESM/ Family.
8.
Along with the photocopies of PPO and other documents also carry the originals with you for verification.
9. The Postal Address of the ECHS Regional Centre Delhi Cantt – II is –
104, Cavalry Road, Maud Lines, Near Blood Bank, Delhi Cantt – 110010.

The Dealing Clerk at the ECHS Regional Centre Delhi –II, is
