Replacement of lost phased out canteen card

Dear Sir,

I lost my phased out card during the period of my wife's ailment and subsequent death. The card was issued by IMA canteen Dehradun. I have already made a police complaint . Presently I am living in a senior citizen home Ludhiana. I rang up by phone to the parent canteen. They want affidavit along with police complaint to be submitted to them. I am 80 now as such difficult to commute. Requirement above for a phased out card appears superfluous since it is not usable.

Kindly advise so that I can have canteen facility for groceries only at Ludhiana.

My CSD smart card has been issued from Jodhpur. Presently I am residing at
Lucknow. Surya Canteen Lucknow states that I have to go to Lucknow in
person. I have all relevant documents available with me. Can I get my card
renewed without visiting Jodhpur? I may kindly be advised.

Yes by all means you can get your csd card renewed from any Canteen. I have done it. My first card was issued by Delhi Area CSD, and was renewed by ESM Canteen, Gurgaon and subsequently by Air Force Canteen at Rajokeri in Delhi.

There is no facility of online replacement of phased out CSD cards. You need to go to nearest CSD canteen where a form is needed to be filled with copy of your discharge book/ pension book. You will also need to attach passport size photo of yours and joint photo with your wife. There are two cards made - one bearing your photo alone is for liquor and another one with joint photo is grocery card.

Retain to acknowledgement slip once application is deposited. Inquire after 15-20 days within period cards would be ready. While collecting these cards you need to carry original discharge/pension papers.

Respected Sir,

With deep pain and agony I would like to state that yesterday i.e. 13/03/18 my father Ex-HONY NB/Sub SADANAND JHA had lost (rather it was snatched) his bag containing all his important documents of ARMY including his canteen smart card and PAN CARD, ADHAAR CARD,etc. We have got the FIR done.

Now he will have to get duplicate canteen SMART card but is it possible to get the same without knowing the previous card number and other details of the lost card.

Kindly advise.
