Issue 35: vxodbc config dialog misleadingly includes unused options

New issue report by apenwarr:
The only config options vxodbc needs right now are the server name and port
number. Username, password, and database name are just misleading options,
so we should take them out.

Similarly, remove all the extra cruft that reads/writes those options from
the odbc.ini.

Oh, and there should be no default name for the DSN; leave it blank as
it should be.
