Final Destination 5 is a 2011 American 3D supernatural horror film directed by Steven Quale and written by Eric Heisserer. It is the fifth installment in the Final Destination film series and a prequel to Final Destination (2000). Final Destination 5 stars Nicholas D'Agosto, Emma Bell, Miles Fisher, Arlen Escarpeta, David Koechner, and Tony Todd, and follows a young man (played by D'Agosto) who has a premonition and saves a group of people from death when a suspension bridge begins to collapse. However, they soon learn that they cannot escape Death's plan.

Sam Lawton is on his way to a company retreat with his colleagues. While their bus crosses the North Bay Bridge, Sam has a premonition that high winds will cause the bridge under construction to collapse, killing everyone except his ex-girlfriend Molly Harper, whom he manages to get across the bridge safely. Panicked, he persuades Molly, his friends Nathan Sears and Peter Friedkin, Peter's girlfriend Candice Hooper, his boss Dennis Lapman, and co-workers Olivia Castle and Isaac Palmer to leave just as the bridge collapses. After being interrogated by FBI agent Jim Block, the survivors attend a memorial service for their deceased co-workers where they are being watched by coroner William Bludworth.

Later, Candice dies during her gym practice from a chain reaction that causes her to fall off the uneven bars and snap her spine. The next day, Isaac's head is crushed by a falling Budai statue during an acupuncture session at a Chinese spa. Bludworth, who has been present for both deaths, tells the remaining survivors that if they wish to cheat Death, they must kill someone who was never meant to die on the bridge and thereby claim their remaining lifespan. At the same time, Sam and Molly fail to save Olivia, who falls out of a window to her death at an eye surgery clinic. Sam learns that the survivors are dying in the order they were meant to die on the bridge and realizes that Nathan is next.

The visual effects were praised for improving on the weak CGI from the previous installment. Betty Jo Tucker of ReelTalk Movie Reviews said in her review "The film boasts some of the best visual effects ever, especially the bridge-crumbling sequence at the beginning of the film." In his review of Final Destination 5, Roger Ebert said "... the special effects do an excellent job of beheading, incinerating, vivisecting, squishing and so on."[40] "Final Destination 5 contain some of the most fun effects ever seen that purely enhance the thrills and bloody spills, rather than detract from them," stated Lisa Giles-Keddie from uk.real.com.[41]

1:40 - Amazing - The zoom in on the bridge - This camera movement could compete with a Lord of the Rings movie in terms of how it shows characters amongst the environment round them. The live action background plates, the people running off in the distance, the subtle swaying of the bridge (the first time we really begin to see this), the slight camera shake that has been tracked to the main plate in the middle (which is perfectly integrated into the CGI, which is almost invisible); gives me goosebumps every time.

3:13 - Amazing - Section sags and barriers slide off - The floor moving beneath the characters is like the anti-star trek bridge wobble. Every scene involving the characters on unstable sections has a believable feel to them. And a shootout to the concrete barriers sliding off, as well as their sound design.

It is the earliest known major disaster of the series, as seen in Final Destination 5, and the second disaster chronologically, given William Bludworth's words[2].

During a corporate retreat, qualified members of Presage Paper travel on Rolland Coach Line Bus 1282 on the North Bay Bridge. Meanwhile, high winds sway the bridge, causing one of its cables to be loosen, presumably done by Death itself. This creates a sudden jolt, which only Sam could feel. Construction workers further weaken the bridge when their machinery causes cracks to form along the surface and the bottom of the deck to break apart.

Sam sees that the bridge is starting to collapse and alerts his girlfriend Molly, his friend Peter and the rest of his colleagues to flee the bus immediately. Candice, Peter's girlfriend, runs to the railings on the right side of the bus. The road beneath her feet collapses and as she clings onto the railings, Candice calls Peter for help. Candice, however, falls off and is gruesomely impaled by the mast of a sailing yacht.

Sam tells a shocked Peter and Molly to leave the scene, as their bus hangs over the edge of the bridge. Isaac, after finishing his phone call in the bus's bathroom, is trapped in the bus, as it falls off. As the bus crashes into the bay below, Isaac is hurled to the windscreen and is killed on impact. The colleagues run away from the site where the bus collapsed and during the moment of chaos, a passer-by knocks Olivia's glasses off, temporarily blinding her. Sam and Molly then witness the middle section of the bridge collapse before them, leaving behind a beam which connects to the other side of the bridge. Sam instructs Molly to cross over the beam, which she successfully does. Olivia, after Sam sees her struggling, is instructed to cross over the beam as well. Meanwhile, Peter tries to reach to Sam but a streetlight falls onto the cars, almost killing him. Olivia is nervous and before she could hold onto Molly's hand, the bridge shakes, causing the beam to tilt backward. She slides off and falls into the bay. Olivia resurfaces but a red car falls off from Sam's side of the bridge and lands on top of her head, killing her. Nathan, and his boss Dennis, run towards Sam but a cable is loosened and violently swings Nathan's body away, blood splattering everywhere which shocks Dennis. Sam's side of the bridge then tilts over, causing many passers-by to fall into the bay to their deaths. Dennis clings onto the edge of the bridge but a truck, containing hot tar, topples over. The tar burns most of his flesh as Dennis falls off to his painful death.

As Sam's side of the bridge begins to collapse, Peter arrives and jumps onto the hanging railings. Sam does the same as the entirety of his side of the bridge collapses, killing everyone who remained. Peter and Sam cling onto the railings, but a truck on Molly's side of the bridge hangs over the edge. The truck contains iron bars and they are soon propelled into Peter's direction, impaling his left eye, abdomen and stomach. Peter's bloody corpse falls into the bay below as Molly desperately tries to reach to Sam. However, the truck hangs over the edge even further, prompting a large sheet of metal to slide out and bisect Sam's body. Molly screams in horror as Sam awakens from his terrifying premonition.

After his premonition, Sam tells everyone that the bridge is going to collapse and brings Molly outside. Despite his colleagues questioning his paranoid behavior, they follow him. This includes Olivia, Nathan, Candice, Peter, Dennis and Isaac, whilst the rest of their colleagues remain in the bus.

Suddenly, high winds cause the bridge to tremble and one of its cables to be loosened. Dennis sees a large crack form on the road and tells everyone to get off the bus. Dennis, Olivia, Nathan, Candice, Peter and Isaac flee to a safe area, where Sam and Molly stayed. Then, they witness the bridge collapse, killing everyone, including those who stayed on the bus. After the bridge collapse, the survivors (which included a few passersby who were later not part of Death's list) look onto Sam, with horror and bewilderment at his ability to predict the tragedy.

Heart failure is a major problem worldwide; it is the leading cause of hospitalisation and is posing a huge financial burden. Advances in healthcare have contributed to increased life expectancy, with a resultant increase in the number of patients with chronic heart failure. For many patients who are still severely symptomatic despite optimal medical therapy and cardiac resynchronisation therapy, cardiac transplantation would be the preferred treatment option. However, hopes are cut short with a limited donor pool of hearts for the increasing number of patients requiring cardiac transplantation. One uprising method to fill this treatment void for patients with advanced end-stage heart failure (ESHF) is the Left Ventricular Assist Device (LVAD). Although traditionally used as a bridge to transplantation, owing to limitation of suitable donors, evidence suggests increasing potential for the use of LVAD as destination therapy (DT), that is, lifelong permanent support. Exploration of DT is a promising avenue to many patients suffering with ESHF who may never be fortunate enough to receive a heart transplant, but not without reservations of its efficacy, safety, effects on quality-adjusted life years and cost-effectiveness, especially in comparison to heart transplantation.

On a pedantic note (as all best notes are), the collapse shown in the film does not accurately reflect the change in shape that the real parabolic cables would take. Without the dead weight of the main span, the main cables would sag noticeably less between the towers because they would only be carrying their own weight. Since the cables themselves cannot appreciably change in length, they would droop much more at the outer spans where the load is still applied. In the movie, the intact portions of the road deck should be sagging much lower. This is a rare fictional collapse scenario for which we have a real world comparison, the 1940 Tacoma Narrows Bridge collapse.
