Our packaged client, when there are enough windows up such that their taskbar icons get 'collapsed' into one - result in this generic-looking icon:
Sorry for obscuring the preview of the windows and the "...Client" blurb in the task bar. But see how the 3 windows are previewed with the correct (balloon) icon (this is with the unmodified nodialog.exe), yet they collapse into the generic taskbar icon you see right next to the tray.
When I was searching for this issue online, I got the impression that this a problem for certain applications (unrelated to Smalltalk), and it even sounds like a Microsoft glitch. Anyone know how to fix it? The problem does not occur in the development environment (even using the same nodialog.exe).

You can try Rebuilding icon cache. To do this, please follow these steps:
1. Click on start and click on computer.
2. On the toolbar click on Organize and choose folder and search options.
3. Click the view tab and check to select the show hidden files and folders option and click ok.
4. Then enter the following directory:
%USERPROFILE%\AppData\Local
5. Find the IconCache.db file, and rename it as IconCache.db.old.
6. Close the window, and restart the computer.
If this does not work then you can do a SFC Scan.
To run the System File Checker tool, follow these steps:
