Form I -821D is the one to file in for consideration of deferred action for childhood arrivals.
Along with this file in Form I-765 ( to request employment authorization) and Form I-765WS
Total fee for all the three forms is $465.
Remember that you cannot file in citizenship application if you are under deferred action
