New results from a large clinical trial provide the first strong evidence that a rare form of ovarian cancer, known as low-grade serous ovarian cancer, should be treated differently from other forms of the disease.
Compared with the most common form of ovarian cancer, known as high-grade serous ovarian cancer, the low-grade form grows more slowly and is more likely to be diagnosed in younger people. Although it is less likely to respond to chemotherapy than high-grade disease, people with the low-grade form tend to live longer after their diagnosis.
Like other forms of ovarian cancer, low-grade serous ovarian cancer is often diagnosed when the cancer is at an advanced stage. And the cancer will eventually come back, or recur, in at least 70% of people with advanced disease, Dr. Gershenson said.

Choriocarcinomaof the ovary (nongestational) is most often found as part of a mixed germ cell tumour. Choriocarcinoma is composed of trophoblast, the same type of cells that form the placenta during pregnancy.
As ovarian cancer is rarely symptomatic until an advanced stage,[43] regular pre-emptive screening is a particularly important tool for avoiding the late stage at which most patients present. However, A 2011 US study found that transvaginal ultrasound and cancer marker CA125 screening did not reduce ovarian cancer mortality.[44] In contrast, a more recent UK study found that up to 20% of ovarian cancer deaths could be prevented through annual performance of these procedures.[45]
Prophylactic salpingo-oophorectomy is frequently performed in carriers of BRCA1 or BRCA2 mutations,[49] although the benefits conferred by this procedure may vary dependent on the specific mutation.[50]
Cytoreductive "debulking" surgery may be performed prior to chemotherapy treatment in order to decrease the physical mass of the tumour and thus reduce the number of chemotherapy cycles needed.[40] The typical advanced presentation as well as extra-ovarian spread seen in HGSC can require aggressive debulking procedures.[52] In some cases total abdominal hysterectomy will be performed, in other cases where the patient intends to bear children a salpingo-oophorectomy is performed instead.

Strategies to increase the effectiveness of endocrine treatment in LGSC are being tested, including the addition of cyclin-dependent kinase (CDK) 4/6 inhibitors (Tables 4 and 5). CDK 4/6, in conjunction with the ER-regulated cyclin D1, plays a critical role in regulating transition from G1 to S phase, and inhibition of this axis is an effective therapeutic strategy in ER-positive, advanced breast cancer. CDK inhibitors (palbociclib, ribociclib and abemaciclib) have been shown to improve survival in women with metastatic ER-positive breast cancer when combined with aromatase inhibitors (letrozole and anastrozole) or fulvestrant (Finn et al. 2016, Hortobagyi et al. 2016, Goetz et al. 2017). Based on these findings in breast cancer, the efficacy of ribociclib and letrozole was tested in a small phase II trial in ovarian and endometrial cancer (n = 40) (Table 5). LGSC patients were among those showing the most significant benefit, with all three women achieving durable responses (one CR and two with partial responses over 2 years) (Colon-Otero et al. 2020). GOG 3026 (NCT03673124), a phase II trial of ribociclib and letrozole; and NCT03531645, a phase II trial of fulvestrant and abemaciclib in LGSC, are currently recruiting, and the results will provide important information on the potential for this approach (Table 4).
Acceptance of a binary histopathological classification dividing serous ovarian cancer into LGSC and HGSC coupled with increased tumour mutation testing and understanding of the molecular drivers in LGSC is transforming the clinical landscape for LGSC and increasing potential treatment options including endocrine therapy, molecularly targeted agents and anti-angiogenesis inhibitors.
The prospect of improving outcomes for women with LGSC has been advanced by a specific diagnosis of this disease. Thoughtful clinical trial design for LGSC, including follow-up of promising findings from early phase trials; rational molecular profiling; novel adaptive platform approaches and inclusion of LGSC patients in basket trials of targeted agents, is now key to improving clinical practice in this area.
