Inearly 2021 Oregon Urology Institute launched an all-new patient portal replacing the system we were previously using. All new and existing patients will register with this new portal by simply filling in the form above with name, email, and DOB.
You will have approximately 5 days to log into the patient portal with your temporary password. After 5 days, the temporary password will expire, and you will need to request a new temporary password.
Patient Portal communication has a number of advantages over other types of communication. It is fast, convenient, and efficient. It also works well for many of the questions, requests, or messages you may have for our practice, such as prescription refill requests, appointment scheduling and non-urgent medical questions.

Once you provide your email address and verbal authorization, a team member will send the Patient Portal Welcome email to you. Your Patient Portal Welcome Letter will contain your temporary password to access the Patient Portal.
The most important thing you should know is we cannot guarantee the confidentiality of exchanges. While the security of e-mail is comparable to other types of communication (such as phone calls), there are some special issues with e-mail.
You should also know that Patient Portal messages you send to your physician may be read by designated staff in our practice. Like phone calls, Patient Portal messages may be screened by office staff before being routed to the appropriate person for a response. Copies of your Patient Portal messages may be placed in your medical record.
Please keep in mind that although secure e-mail can be a very effective tool, it is not a substitute for a physical exam by your physician. Please call the office if you feel you need to see your physician.
Every effort will be made to respond to our Patient Portal question or message by the next business day. You should not expect to receive a response on weekends or holidays or any other time the office may be closed.
Please be aware that if you are not currently active on the Patient Portal, you will need an activation code. If you would like to activate your portal account, please contact your office to receive your activation code.

The previous Patient Portal will still be available for a limited time for historical purposes, however, you must use MyMedicalLocker to create new appointments, keep track of vital information, and communicate with the practice.
The patient portal allows you securely access your health information and better manage your care. Having direct access to your health records (including labs and clinical summaries) and the ability to update contact details, allergies, medications, and other important health information will allow you to take greater control of your care an increase the the accuracy and communication between you and your doctor.

The Patient Portal is a personalized, secure on-line way for patients to gain access to their individual medical record. To update your demographic, insurance, or pharmacy information click on Account Info. To change your email, password, login, or security questions, go to Edit Profile at the top right corner, underneath your name.
For patient security, the access code you are given will expire after 30 days and is no longer valid after you have used it the first time. You will need to contact our office to be given a new access code if the one you have is not working.
