Survey format for RMSA

Dear friends,
As you know that the RMSA consultation has been concluded at Lucknow on 5 January 2015 and there came some important suggestion towards further action. After that we have discuss the same in secretariat meeting and decided to include the more information. So, we are sending all of you a survey format on the behalf of our alliance VOP to complete the report of RMSA with attachment. It would be applied only govt.secondary schools of your respective areas. So,we are requesting you kindly collect the data and information by 11 February 2015.
