Splitting a Jack

We need to have an ethernet jack split for two computers in one room. I
found the Jack activation form, but I'm not sure how to request this
service.

Any help would be appreciated.

Just request an ethernet connection to the jack in question and type
"split" in the comments field.

Just request an Ethernet connection in that room and in the comments
say that this is a request to split the jack.
