I'm seeing this about 1 out of 10 times during boot of RT-11 on my Heath
H-11 system. Does anyone have an idea what the underlying problem could
be?

FYI, I just saw the same thing trying to shut down VMS 5.1 on my MicroVax with a QBone.

I see that almost 100% of the time when attempting to shutdown my QBone.
