Emailemail protected] Put your first and last name in the subject line. In the event that you need to confirm receipt of your e-mailed medical certificate, DMV will track it using the information in the subject line.

Federal regulations require that every commercial learner permit (CLP) holder or driver license (CDL) holder certify to the type of commercial driving they currently perform (or expect to perform).

The DMV will send CDL/CLP holders a "Notice of Non-Compliance and License Downgrade" if their medical certificate on file expires resulting in their medical certification status becoming "Not Certified." CDL holders will be downgraded to a non-CDL class if an updated Medical Examiners Certificate (MCSA-5876) is not submitted and added to the drivers record within 55 days of the letter date. CLP holders will have their permits cancelled if an updated Medical Examiners Certificate (MCSA-5876) is not submitted and added to the drivers record within 55 days of the letter date. Employers that subscribe to the NY State License Event Notification Service (LENS) can sign up to receive notification of these status changes. Passenger carriers enrolled in the 19-A system will also receive status change notifications.

If you have a commercial driver license, Class A, B or C or CDL permit issued by the NY State DMV you must certify the type of driving you do anytime you perform a license transaction (other than an address change, or duplicate license). You will be required to choose only one driving type from the four federal driving types listed below.

Note: If you cannot meet the criteria for one of the driving types defined above, you can no longer hold a CDL. You must go to a DMV office and downgrade to a non-commercial class driver license.

DMV will send notifications regarding CDL/CLP downgrade/cancellation events to passenger carriers enrolled in the 19-A system. Drivers can also monitor their own CLP/CDL medical certification status through MyDMV and subscribe for text and/or email notifications.

If your USDOT Medical Examiner's Certificate indicates "must be accompanied by a Diabetic, Hearing, Seizure or Vision waiver exemption or an SPE Certificate," you must have a Medical Variance issued by FMCSA.

To avoid rejection, please complete the correct form (MCS-5876). The form number is located in the top left-hand corner of the form. The long-form physical (MCSA-5875) is not a substitute for the certificate and cannot be used. Also, ensure the medical examiner completes and signs the form. The driver must also sign.

Effective January 1, 2016 the IRP Plan removes the exemption for charter buses. All IRP jurisdictions will start registering charter buses if they travel interstate to other IRP jurisdictions.

Whether they are Excepted or Non-Excepted, all CDL holders must submit a Self-Certification Affidavit to the Tennessee Department of Safety and Homeland Security when they apply for a CDL, apply for a higher class, apply for a new endorsement, transfer a CDL from another State, or renew their CDL.

Interstate commerce is when drivers operate a commercial vehicle from one State to another State or foreign country, or that passes through another State or foreign country while traveling from one location to another within the same State, or that transports cargo that is part of a trip that began or will end in another State or foreign country.

If a commercial vehicle and its cargo are only driven within a State, that is Intrastate Commerce. Drivers who transport both intrastate and interstate commerce, must choose interstate commerce.

Commercial vehicle drivers who ONLY transport intrastate commerce that does not require a hazardous materials, passenger, or school bus endorsement do not need to have a Medical Certification. If you

transport both excepted and non-excepted intrastate commerce, you must maintain a current Medical Certification.

Based on your self-certification, you may need to have a physical performed and have a Medical Examiner's Certificate (Form MCSA-5876) completed by a certified medical professional. The medical professional that performed the physical is required to send a record of the certification to the FMCSA electronically. However, the CDL holder is responsible to notify the state.

Drivers must submit their Self-Certification Affidavit and if they are Non-Excepted their Medical Examiner Certification (Form MCSA-5876) to the Tennessee Department of Safety and Homeland Security using e-Services.

To renew your Self-Certification and Medical Certification, you must have a new physical performed. Submit (upload) your Self-Certification Affidavit and Medical Examiner's Certificate to the Department of Safety and Homeland Security five (5) business days prior to the expiration of your current medical certificate. If you have a variance, you are also responsible for applying to renew it with FMCSA.

If your medical certificate or variance has expired or will expire before you can provide the Department of Safety and Homeland Security with a new one, all CDL privileges will be removed from your license. If your medical certificate will expire before you can renew it, you may be able to change your self-certification to an operating category that does not require a medical certificate.

Once your medical certificate has expired, to get back your CDL privileges, you will need to reapply for a medical certificate which may require retesting and additional fees. If your variance expires you must renew it with FMCSA.
