1. g bar council, beli borang petisyen penerimaan masuk/chambering @
CARIK la kat lawfirm kau kot2 diorang de simpan 1. RM 5 je
2. kalau beli kat bar, die ade bg 1 buku panduan
3. Isi la borang tu.. Isi borang 1,2,3,4,5 dulu.. 6,7,8 tu utk long
call..
4. ikut sumer details tu.. pastikan xlebih line time isi, xlebih ruang
etc. TULIS BIAR JELAS, BOLEH BACE DAN xmerapu.
5. sign la mane yg ptt.
6. pastikan master sign borang 4.
7. xperlu amek sumpah lagi.
8. release letter, IC dan birth certificate g photostat.
9. pastikan salinan photostat td da di CTCkan.*CTC= certified true
copy.
10. pegi court, tingkat 2, pendaftar mahkamah tinggi sivil, bahagian
rayuan dan kuasa-kuasa khas. - jumpe mr. murali. die akan bg arahan
seterusnye dan check kan borang 1 dan 2 tu dulu dan CTC photostat td.
11. die akan suruh betulkan if ade salah, so make sure ade salinan
beberapa set borang-borang td yang kosong.
12. die akan suruh g bwh amek sumpah kat bwh (RM4) + g photostat lagi
die tingkat 5 hujung ( rm 5.60 ) + g bayar pulak RM118 kat bwh jugak.
13. pastu die akan cakap pasal borang 3,4,5 dan cara isi dan filing.
14. if ok, blh file on the same day - tp kene g amek sumpah dekat
medan selera.( rm8)
15. die akan suruh g photostat lagi 2 set borang 3,4,5 (rm1.20)
16. pegi la bayar rm3.60 di bawah dan sumer settle.
17. if cepat, maybe on the same day dah boleh pegi serve sumer tu
kepada AG, bar Council dan KL Bar ( or any respective state bar )

I just filed my Form 1-5 today....
really thank the clerk in my firm to help me type the borang.
total i spend :
Form 1 and 2
photocopy form 1, birth cert, ic and release letter 4 set + borang 2 -
32 set
Rm 5.6
Angkat sumpah(press button "A" before proceed to counter 22 and 23 i.e
the angkat sumpah counter)
Rm 4
for them to help me serve to respective court
Rm 118
Form 3-5
Angkat sumpah at medan selera -
Rm8
Photocopy 2 set each of the form
then pay RM 36 for dunno what (i think for them to serve for me,not rm
3.6)
then go back after 3-4 days to check which court u will go...
so total sending is RM171.6
keep the receipt , can claim from your firm.
the counter for admission to become A & S is called kauter Bahagian
Pendaftar Rayuan @ Kuasa- kuasa Khas. (if i am not mistaken)
the location is at second floor, is at your left hand side if you walk
in from main entrance.

be reminded the counter 's working hour :
Monday-thu 8.30 -12.30
2-330pm
Friday - 8.30-12.15
2.45-3.45pm
use black color ink. like what Fahrul say, dun forget to ask your
master sign borang 4. bring some extra blank form just in case. and
remember to smile = )

P.S- just found out that everyone, even you chamber at Kedah, still
have to file their paper at KL Court when long call. someone i met in
the court told me. not sure about short call. Haha...

and after you get ur Borang 1 & 2, u have to serve it to AG, Bar Council and respective state bar. where u do ur chambering. for eg. like me, jamme, shikin - we serve it to KL Bar.
and, later , when u get your borang 3,4 and 5 u have to serve it also.
and after u have serve it, u have to prepare a "AFFIDAVIT PENYAMPAIAN" for Borang 1,2,3,4 & 5.

Certified true copy mean document that "disahkan" . You can ask your
master to certifiy for you, or any other lawyer in the firm will do.
they have 1 cop .make sure they sign Borang 4.
about borang 4 's angkat sumpah, you will need to go to the
commissioner of oath at Medan Selera. I think can't do in the court
because your master is not present with you. so , settle it out of
court.
prepare some blank form just in case you make some mistake and need to
refill some page.
Borang 1 and 2 only need 1 (ONE) set of birth cert, IC (back and front
on the same page )and release letter.
