Despite Mario Kart moaning about screen resolutions, does it actually make a
difference to gameplay - especially online play, or has the actual game been
optimised to run at the same speed regardless of setting?

I prefer the higher resolution of 576i via component, because as someone
else pointed out to me my HDTV deinterlaces better than it upscales.

I've been using 480p as default, since I'm running the Wii into a Dell
widescreen TFT. I've not watched any Nintendo channel vids, but
in-game videos are almost invariably 60Hz based and therefore a little
jerky on 576i.
But! 480p *seems* to cause trouble in Lego Indy, scripted bits not
completing correctly and wedging the game. Although it might be
coincidence that I haven't had any after experimentally going to 480i.

I'm on 480i. I just tried 576i and the Mario Kart in-game frame rate
was a mess. The frame rate was inconsistent which to my eyes is more
offensive than a low frame rate, especially in a racer. I switched
back to 480i and it started running as smooth as butter again.
I'm not sure what you mean about Mario Kart "moaning" though.

Ah, for some reason on my Wii, I get a popup message at the start suggesting
that I switch to 480i or 480p for better perfomance. For some reason this
seems to happen every time I start up the game unless I actually switch
resolution.

Perhaps Paul uses component? If you use RGB or composite, it wouldn't
tell you to do that as it couldn't be sure your telly could support
the NTSC modes (not that many don't these days).
Truly bizarre. I wonder if you have a later version of the game?
i've only had it appear once, iirc, but i've definitely had it.
