I'm new to php and would appreciate a REAL example of how to include an
entire form (with at least 3 fields) into a session variable to be be
recalled at any time.

I've read the definitions of many session vars, but I've not been able to
see real concrete examples of how to use them on simple forms and parameter
passing.
