PROCEDURE FOR RENEWAL OF EXPIRED SLA LICENCE

Dear user,

 

As per copy of license attached date of expiry of license 22.03.2014 and as per IRDA regulations renewal of license is allowed with in six months of date of expiry of license. In such case, your license falls under Lapsed License category and you have to apply offline with below mentioned documents. 

 

The Authority has decided to exempt from training and examination to those applicant whose surveyor license has been lapsed as per Section 7(1) of Insurance Surveyor Regulations, 2000. The Authority will grant the license with “C/Licentiate” category and eligibility of department may be decided on the basis of qualifications as prescribed in notice no. 04 dated 11.02.2002 for insurance surveyor and loss assessors. Application may be filed in offline mode with the documents mentioned below:-

1.    Demand Draft for Rs………../- (1000/- plus applicable Service Tax @ 14% ) in favor of         

Insurance Regulatory And Development Authority, payable at Hyderabad.

2.      Copy of Degree/Diploma attested by Notary/Magistrate only.

3.    An affidavit on Rs.10/- Stamp Paper as per the format given against Sl. No.3 of the application from IRDA-1-AF duly notarized (including all the 6 clauses) may be sent to this office.

4.    Additional affidavit should be made mentioning following:-

a.       There is no disqualification to act as surveyor and loss assessors

b.      There is no disqualification as provided under Section 42D of the Insurance Act from the date of expiry of license SLA…………(surveyor license number) to till date

5.    Self-addressed envelope of 4.5”X10” (As regards amount of stamp for speed post, please check with postal department and affix stamp accordingly).

6.    The Form 1-AF placed on our website www.irda.gov.in may be filled in duly, completed and sent to this office.

7.    No Objection Certificate from your employer for grant of license if applicable.

8.    Copy of PAN Card.

9.     Address proof (driving license, passport, etc.)

10.  Copy of IIISLA (Indian Institute of Insurance Surveyors And Loss Assessors) Membership.

 

      Original license(lapsed) to be surrender with document
