Smoking status and Tobacco Use sections in CCDA

We have sample PDF files provided by ONC which contains data as below:
Element Description : Smoking Status
[ Description ] : Heavy tobacco smoker
Start Date : 5/1/2005
End Date : 2/27/2011
Code : 428071000124103
Code System : SNOMED-CT

We have a question in this for tobacco Use section
Q) What exact value is expected in tobacco use entry template as detail code is not given?

Q) What effective time value is expected in Smoking Status meaningful Use?
As only start Date and End Date is given in samples
