Intermittent login problems ?

We've had a recent uptick in cases where a user cannot login (shows bad password) and eventually gets locked out. In at least one case we fixed it by setting a new password. No errors on the backend except for registering as an incorrect password. I know it's not much to go on and it might just be a fluke, but I thought I'd post it here in case it rings any bells for anyone. Thanks!
