Provide a brief introduction that includes the mission statement, vision, and/or the goals of your practice learning site. Answer = Atlanta VA medical center. There mission statement on the internet.


Compare the demographic data of Sentinel City with the demographic data from the city where your practice learning site is located. You should present this information in a clear to understand table. You can use the Data Comparison Table provided in Module 2. Compare sentinel city to Gwinnett county Georgia


Identify 3 key informants in the city where your practice learning site is located and 2-3 questions you would ask them. Examples of key informants include, but are not limited to: political leaders, health care providers, service providers such as mail carriers or shop owners, members of the fire or police departments and long term residents of the community. People who live and work in the community are a rich source of information and can often provide data not found in printed form. Your mentor from your Practice Experience will serve as the 4th key informant. Include specific quotes in response to the questions you asked your mentor that support and/or contradict information cited in the literature.


Present a list of the top 3 health concerns for your target population. Provide a brief description of these concerns and include data from you target population city that supports the rationale for selecting these health concerns. Select one of the three health concerns you identified and state why you decided to develop an intervention that addresses this particular health concern/issue. Include the Healthy People 2020 objective that relates to your identified health concerns. Answer: hypertension, diabetes, and Heart disease


Develop a community health nursing diagnosis based on your analysis of the demographic and health status data that you gathered, as well as your conversations with the key informants. You may consult with your practice learning site mentor to help you develop this diagnosis.


Describe your proposed intervention that will address the community health nursing diagnosis for your target population. You may choose to develop your own intervention, or modify one that was recommended by the Task Force on Community Preventive Services (TFCPS).


Whether you modify an existing intervention or develop one of your own, you must discuss why this intervention will or why you needed to modify it because it would not work for your target population. Include advantages, population fit, and barriers in use of the intervention. You should include the long-term goals, two measurable objectives, and the required resources such as: time, equipment, finances, etc. [Note: you are not required to carry out your intervention but may present it to your practicum site mentor]


Describe your method for evaluating the objectives of your intervention (for example, pre/post-test, survey, questionnaire, phone interview). Include one long term goal and two short-term measurable objectives.


Summarize the assets (strengths) in Sentinel City pertaining to the 8 community subsystems. In addition to the "people" (the core) of the community, there are eight subsystems that come together to form the assessment data for your windshield survey, which will contribute to your overall community assessment of Sentinel City. The eight subsystems within every community include: 1) physical environment, 2) safety and transportation, 3) health and social services, 4) education, 5) recreation, 6) politics and government, 7) communication, and 8) economics.


Include your recommendations for improving/strengthening specific subsystems in Sentinel City. Discuss other recommended changes or additions needed to improve the health of the population living in Sentinel City. Answer: Find the information on the internet


Include a minimum of 6 references in APA format. The reference list should include relevant websites from state or federal sources and from peer reviewed professional journals. Journal articles do not have to be limited to nursing journals. No more than 50% of references should be from websites.


Basic Windshield Survey - RuralEnter Sentinel Town via the link to begin your virtual experience by taking a tour by car of Sentinel Town. Since this is your first tour, select the slowest speed and stay in the car the entire time at least once before getting out of the car. As you take the tour, write down your observations, specifically those that align with the following demographics and/or subsystems listed in the left-hand column of the rubric. On May 11, 2023, the U.S. Coast Guard advised the City of Chesapeake that a spud barge struck the underside of the southbound span of the Rt. 168 Bypass Bridge, damaging one of the girders underneath the bridge. On May 12, City crews and structural engineering consultants inspected the bridge and confirmed the damage, then closed the outside lane.


Yes, the bridge is safe to drive on and engineers are continually inspecting it to make sure it stays that way. The single lane closure helps to relieve additional weight from the affected area to avoid causing further damage to the structure.


Fixing a bridge of this kind is going to take specialized professionals who will likely need to travel from out-of-state. At this time, we do not have an exact timeline for repairs, but it will likely take several weeks to a month. Contractors are already working on design plans to prepare for the repair.


UPDATE: The U.S. Coast Guard has granted the City's request for limited bridge openings at the Great Bridge Bridge. The Great Bridge Bridge will remain closed to waterway traffic (open to road traffic) Monday-Friday, 7:00 - 9:00 a.m. and 4:00 - 6:00 p.m. At all other times the bridge will operate as normal. Please note that federal regulations require that the bridge open at any time for red flag vessels so this could still lead to bridge lifts during those periods.


Tolls on the Veterans Bridge go towards repayment of the bonds that were used to fund construction of the bridge and there are strict guidelines around when the tolls can and cannot be lifted. The bond indenture precludes lifting tolls in this situation.


Traffic Engineering staff are regularly monitoring and adjusting the traffic signals on all of the alternate routes. Adjustments are being made both remotely and on-site. Keep in mind that traffic signal adjustments can only have a minor impact on overall traffic flow.


The City is working alongside our engineering consultant to determine the necessary vehicle weight restrictions on the bridge during the repair project, as well as developing traffic plans to allow as much traffic as safely possible on the bridge when no work is underway. Out of an abundance of caution while these determinations and plans are being prepared, we have closed the outside lane of the southbound span to traffic until our analysis and work plans are complete. More information on changes to the traffic plan, including the potential to reopen the second lane at select times, will be released as it is confirmed.


When setting up lane and road closures, traffic engineers must follow regulations from the Manual on Uniform Traffic Control Devices (MUTCD). Because of the speed and configuration of the road at that point, the closure has to start that far back.
