From what I have seen this is only possible on a full directory basis ("mark
for comparison" one dir then "show differences as unified diff").
We then have to awkwardly copy/paste to a new file rather than being able
to save the output.
TortoiseMerge allows us to *see* the unified diff but not save it (or
copy/paste it).
I am using TortoiseSVN 1.5.6 with a 1.4 server (for now).
Is there a way to get what I want that I have overlooked?

Yes it does:
Menu "Edit->Create unified diff file" will ask you for a location to
save the unified diff file.
