TOPS Families:
The Seattle Schools and Seattle Fire Department COVID-19 vaccination clinic for students and family members 12 and older is this Friday, May 21 from 2:00-5:30PM, in the gym. Please go to the courtyard to line up for entry to the clinic.
Students will need a signed parental consent form. Families can return consent forms via:
- Print, sign, and return paper form with student attending clinic. https://www.seattleschools.org/district/calendars/news/what_s_new/covid-19_vaccine
- Accompany student to clinic to give written or verbal consent.
- Email jebion...@seattleschools.org if you need to give electronic consent. Email should contain the following: “I, parent name, authorize my child, student name, to receive the Pfizer Covid-19 vaccine.”
