The film is the fourth collaboration between Barjatya and Khan after their previous films Maine Pyar Kiya (1989), Hum Aapke Hain Koun..! (1994), and Hum Saath Saath Hain (1999), and it is the second collaboration between Khan and Kapoor after Saawariya (2007).

The film tells the story of Yuvraj Vijay Singh (Khan), a wealthy crown prince of Pritampur who is soon to be crowned King. He is engaged to Rajkumari Maithili Devi (Kapoor), a princess of the royal family of the neighbouring kingdom Devgarh. However, due to his stiff and stubborn nature, Vijay faces many issues with Maithili and his siblings. His half-sisters, Rajkumari Chandrika (Bhasker) and Rajkumari Radhika (Bhatia), live in a separate rented bungalow outside the royal palace and have filed a case against Vijay for a share in the royal property, because they felt their mother was wronged by the Queen when she disgraced her in front of all of the King's children. Furthermore, Chandrika and Maithili, who were best friends during school times, find their friendship shattered owing to her engagement with Vijay. Similarly, his half brother Yuvraj Ajay Singh (Mukesh) vows vengeance and has been wanting to kill him and take the crown for himself. He enlists the help of his manager Chirag Singh (Armaan Kohli) and Vijay's secretary Sameera (Samaira Rao), who are secretly working against him.

Four days before his coronation, Vijay survives an assassination attempt by Ajay and Chirag. He is airlifted to a hospital for treatment, where he meets Prem Dilwale (Khan), a carefree stage actor who looks exactly like him. Prem is a big fan of Maithili and had planned to meet her at an event organised by his friend Kanhaiya (Dobriyal). Vijay's loyal bodyguard Sanjay (Deepraj Rana) asks Prem to impersonate Vijay until he recovers from his injuries. Prem agrees and undergoes training to act like Vijay.

Prem enters the royal palace as Vijay and tries to mend the relationship between Vijay and his family members. He also falls in love with Maithili, who reciprocates his feelings. However, he faces many challenges from Ajay and Chirag, who are plotting to expose him and kill him. Prem also learns about Vijay's past and how he became cold-hearted after losing his parents in a bomb blast orchestrated by Ajay.

On the day of the coronation, Ajay kidnaps Vijay from the hospital and replaces him at the ceremony. He reveals his true identity to Maithili and tries to force her to marry him. Prem arrives with Sanjay and Kanhaiya and fights with Ajay and Chirag. He manages to rescue Vijay and expose Ajay's crimes to everyone. Vijay forgives his siblings for their mistakes and reconciles with them. He also realises his love for Maithili and asks her to marry him. Maithili chooses Prem over Vijay, saying that she loves him for who he is. Vijay accepts her decision and blesses them. He also decides to abdicate the throne and give it to Ajay's son Samar (Rakesh Bedi), who is a kind and honest boy. Prem and Maithili get married and live happily ever after.

Prem Ratan Dhan Payo was released worldwide on 12 November 2015 during Diwali season. It opened to an extreme positive reception with praise for Khan's performance, background, visual effects, story, screenplay and direction. With a worldwide gross of 432 crore, it is the second highest-grossing Bollywood film of 2015 and the 20th highest-grossing Indian film.
