Stuck in insert mode (overwrite mode) in Tmux

I'm doing something accidentally that makes me stuck in insert mode
while in tmux. I have no idea what I'm doing or how to set it back.
Outside of tmux not a problem.

Pressing the insert key does not fix it.

Tried in various ssh clients: bitvise, putty, firessh. The problem is
localized to a particular tmux session. If I start a new session, no
problem. And the problem is in all panes of the particular session.

Or, could it be a server issue?

Any program. Even the command prompt. For instance, I can type aaaaa,
back-arrow to the beginning, type bbbbb, and it writes of the a's. Like
the behavior you get when you press the insert key.

Just one tmux window or all? What if you attach in a new terminal? What if you restart tmux?

I only use one window at a time, but multiple sessions. It happens only after I've used the session a bit, say 20 minutes. It does not affect other sessions until I've used them for a while as well. It especially happens when I'm using multiple panes. If I start a new session it does not start until I've used it.

I've isolated the problem some more. It starts when I use emacs, and mostly when I have more than one pane. Typically, this is what happens:

tmux new-session -s test
C-%
emacs test.txt

type some stuff
go back to beginning of line
type some more and it overwrites
exit emacs
now overwrites at command prompt as well

My terminal is xterm-256color. I just tried a number of others to see if that changed anything, but it didn't.

If your TERM is xterm-* inside tmux then that is wrong.

What terminal should I be using in tmux? It's nice to have color.

So, this is interesting--I deleted my .emacs file and .emacs.d directory (both empty) and now I'm not having the problem any more.
