Just out of curiosity. Is it possible to upload an image or only insert one locally hosted?

If you drag and image file into your wiki it will be stored as part of the wiki. Be careful doing this because it will quickly make your wiki large.

Thanks for hint. Would be good however to have some indication of upload possibility.

What do you mean "upload possibility". Upload may be the wrong word here. Dropping an image on tiddlywiki imports it into tiddlywiki. You can also reference external images or local (relative to the file) eg images folder.
If you are using a NodeJS Served version of TiddlyWiki things work differently too.
