Is there a plugin for building questionnaires?

Is there something available to do something like a questionnaire to capture information in short/long answer, multiple choice, linear scale, checkbox list format? Similar to google forms?

Alternatively, any ideas in this direction?

Hi passingby,
I guess all logic for the questionaire is in TW
Connecting the Checkbox-widget, Inputfields and some other exisitng things like for example https://tobibeer.github.io/tw5-plugins/#rate you can make an elegant questionaire.

The problems will come up when trying to make the clients export their data and get the results back into TW.

If someone has an elegant solution for this I would also be interested.
