Blood Glucose response to Chicken Pulao & Mango
Most of our CGM documentation has focused on the glucose response to 100 gm portions of different breads. However, in a typical meal, a person—whether diabetic or non-diabetic— consumes more carbohydrates. For example, a large plate of rice that is enough to satisfy hunger can easily contain 400–500 g of cooked rice, and many people also eat fruit or a sweet dish after the meal.
The diabetic patient (a family member) consumed Chicken Pulao containing 300 g of white rice and 100 g of chicken, followed by 100 g of fresh mango. The resulting blood glucose spike was much higher than expected. The pre-meal glucose level was 135 mg/dL, which increased to 277 mg/dL two hours after the meal—a SPIKE of 142 mg/dL, & I immediately understood that glucose would remain elevated for 6 hours.
The patient had taken 120 mg of Diamicron MR at 10:00 a.m. with breakfast. By lunchtime at 2:35 p.m., the medication had helped keep glucose at 135 mg/dL, which was a relatively controlled reading. However, the high-carbohydrate lunch produced a substantial glucose excursion despite the medication already being active.
Diamicron MR is a modified-release medication designed to provide glucose-lowering activity over an extended period (several hours). However, a medication does not necessarily prevent a significant glucose rise after a meal containing a large carbohydrate load. Once blood glucose rises substantially—particularly above 250 mg/dL—the subsequent decline can be much slower.
