With the permission and Tawfeeq of Allah, I have been performing Hijamah for the past 17 years.

Based on my personal experience, I believe that draining a significant volume of blood may produce better results in some cases. I have discussed this subject previously, sharing blood test reports comparing lipid profiles before and after Hijamah. In particular, I highlighted the considerable reduction in triglyceride levels observed in the cases where approximately 250–300 ml of Hijamah blood was drained.

As part of my practice, I measure the volume of blood collected during each session. For male patients, I generally stop the procedure once the collected volume reaches 300 ml, while for female patients, my wife drains a lower volume, considering 150 ml sufficient. These figures reflect my personal experience and should not be interpreted as universally recommended targets.

I generally perform Hijamah with the patient in a sitting position, as I have observed better blood drainage in this position, compared with a patient lying down. In my experience, performing Hijamah with the patient lying on their stomach (face down / prone) is the worst position & least effective in terms of blood drainage, although many Hijamah practitioners commonly prefer this position.

Many practitioners make very superficial incisions, sometimes in accordance with the patient's preference. However, I believe it is also worth examining the tools available during the time of Rasoolullah ﷺ. The blades used in those days differed from the fine surgical blades available today, and their design may have influenced the depth and nature of the incisions.

Alhamdulillah, I have observed positive outcomes in my practice, with many patients reporting relaxation after Hijamah. I have also seen significant reductions in elevated blood chemistry markers in some patients' follow-up blood tests.

I am sharing these observations from my 17 years of practical experience to encourage further discussion about Hijamah techniques, incision depth, patient positioning, and the volume of blood collected.

Ultimately, these observations represent my personal experience. Further systematic research is needed to establish how these factors influence outcomes and to determine the safest and most effective approach for different patients.

The blood volumes I mentioned apply only to adult patients whose hemoglobin levels are within the healthy range.
