Keeping in touch with past residents?

Heres something I could use some ideas about....as a part of the
statistics we all collect about our clients and their progress we need
to be able to contact them after they have exited and find out how
they have been doing.

How do you all do this?

So far the most successful method I have come up with has been our
program's Facebook page. Cell phone numbers change too quickly, as do
addresses. But even this method doesnt get me in contact with the
majority.

I would love to hear how other people are handling this!

We have created a resource center that is warm and inviting with
computer access, laundry facilities, and snacks. About half of our
former clients keep showing up, invited or not. The other half are
contacted by their case managers via email, etc and are taken out for
a meal at the 6 and 12 month follow-up marks in order to complete
follow-up reports. Our alumni are invited to continue participation in
any of our events, which I think helps as well. Hope this helps!
