Bonjour, hi,

Each "company" needs to take 2 minutes to fill out this form and send it back by email. I will print out the filled copies when i leave the office this evening.

The representative must be the founder with the major equity portion for corporations, or the significant partner for common name societies.

Please note thar proxies and delegates are not accepted at this time (we are startups after all).
