Hello Colleagues and Professor,
Here is some information suited for 9-12 grade students.  I also attached the file if you would like to download it that way.
 

Interest Survey – “STUDENT INTEREST SURVEY”  Grades 9-12

This site offers a variety questions that will allow educators to learn more about the interests of their students.  Many of the questions focus on the students’ personal interests, which would help guide classroom instruction.  A teacher could use particular prompts like “I would like to learn more about…” to gather ideas on topics that might engage their students.

 

Learning Styles Survey – “WHAT'S YOUR LEARNING STYLE?”  Grades 9-12

Teachers could utilize this site to identify a student’s strengths and weaknesses.  The online format allows students to complete the survey, submit answers, and print results.  If teachers know how to reach students by their learning styles, it will help them design curriculum and differentiate learning.  As Throne (Laureate Education Inc., 2009a) mentions, teachers often teach from their own particular learning styles, which can cause problems for students that learn a different way.  This site could help us pinpoint areas in our teaching that we could improve.

I like this site because of the drop down buttons.  Students answer each question using a 1-4 rating system.  If the results indicate a student learns visually, a teacher may need to consider using samples of work, proving handouts, and incorporating videos to aid with understanding.

 

Multiple Intelligences Survey – “MULTIPLE INTELLIGENCES INVENTORY”  Grades 9-12

The results from this survey can help educators learn more about students’ strengths.  This information will help teachers hone in on those strengths and give students more opportunities to use them in a classroom environment.

The site includes the charts to calculate the results and determine which intelligences are more prominent for each student.

 

Learning Profile Inventory – “TONI ERWIN’S INTEREST AND BACKGROUND INVENTORY”  Grades 9-12

If a teacher would like to learn some background interests about a student or learn more about what they already know coming into the classroom, a survey like this one could provide that information.  I have never used surveys before, but I can see how they might provide enough information to help an educator make important decisions on where to begin teaching.  For example, if a teacher discovers that the majority of her students have a lack of self-confidence, she might need to provide warm-ups or anticipatory sets to build into an activity to avoid student anxiety and frustration.

The survey could be modified to fit any classroom.
