Interest Survey - http://printables.scholastic.com/printables/detail/?id=35689&ESP=PRT/ib/2010/acq/deeplink////

The interest survey I discovered is on Scholastic.com.  It is primarily designed for elementary students, but could be modified for middle or even high schoolers.  This survey focuses on general questions about the students and their interests.  It also has many questions concerning their thoughts on school.  It is a nice way for the teacher to learn about their students, and also a way for students to reflect on themselves and their interests.

Learning Style Survey - http://www.edutopia.org/multiple-intelligences-learning-styles-quiz

There are four sections of questions, each focusing on a different aspect of the taker’s personality.  The results are broken into categories that appear to be based on Gardiner’s Multiple Intelligence theory, to recommend what intelligence or learning style the taker is more attenuated towards.  In my estimation, the language on the survey suggests it is designed for high school and college level students.  I think this survey would be great to get a basic idea of what approaches would reach the most students in your classroom, allowing you to plan accordingly.

Multiple Intelligence Survey -   http://www.bgfl.org/custom/resources_ftp/client_ftp/ks3/ict/multiple_int/questions/questions.cfm

This survey takes the form of rated questions.  The questions are mainly about the interests of the survey taker, and how much they agree with certain statements.  The results are in the form of a graph, again broken into eight of the multiple intelligences.  The questions are worded very simply, possibly in order for use with elementary students.  The survey could be used with students up to and including high school as well.  Like the learning style survey, this could be used to find which approaches best suit your class overall.  I think it would also be great for the students themselves to find out a little about how they learn, so they could reflect on how to become better learners.

Learning Profile Inventory - http://www.tecweb.org/styles/canfield1.html

This inventory consists of questions with four answers, and the taker is asked to rank those answer from 1-4 in accordance with their reactions/feelings.  The questions are generally descriptions of student reactions to classroom situations such as content, grading, teachers, and subjects.  This survey is appropriate for middle/high school level students.  The results are very mathematically presented, which to me says that this is more for an educator to interpret and share rather than the students to interpret on their own.

Hi David,
Sorry I missed your e-mail at the beginning of the week.  I'm glad we were able to figure this out.  Thank you for the resources.
