I have been working with project managers that cannot bear to submit an exception report. Whenever they find out they will only deliver half of the result or we need another million to actually get things working they insist to submit a "decision paper" to the steering board. The oblicated options section in the template scares them as one of the better options might be to better kill the entire project, I guess.
