This is first class work and I am very excited to see this. I know similar work has been done in several other provinces, but these two researchers have taken the extra time and effort to analyze their data and publish them. This sign of taking ownership of the epidemic is fundamental if we ever want to control it. Congratulations to Drs Worapong and Vichai from Pak Chong International Hospital and the Nakorn Rachasima Provincial Public Health Office with this achievement.
Despite the uniqueness and importance of this report, I think the conclusions could benefit from a further and more elaborate analysis of risk. I am particularly interested in the geographic location of sexual behavior, and in characteristics of sex partners. I have no intention of stereotyping, but word on the street has it that during adolescence and early adulthood, several young men and transgender from Korat engage in the entertainment sector of Pattaya and Bangkok for shorter or longer periods of time. This work usually involves a more or less mandatory availability for sexual services, and more often than not also includes sexual contacts with co-workers, friends, ex-customers, and others.
My thinking is that a substantial number of the HIV infections seen in this population have their origin in sex work or in the sexually charged environment that surrounds it.
Another remarkable figure is the more than 40% of MSM with a history of sex with women. I do not know whether this includes sex with TG, but given the average age of MSM of 22.6 years and the cultural limitations for young women to have consensual sex, I am assuming that at least some of the sex with women has been with female sex workers. The HIV prevalence among direct sex workers in Korat has been around 6%-8% in the past 5 years or so, and it is possible that some of the HIV infections were acquired from that group.
A multivariate analysis of these and other factors would quickly show whether any of these factors is associated with HIV infection and this would be important information to decide where prevention should be targeted.
