The Shared Questionnaire System (SQS) is doing something here under Apache license:
in Java, XSLT and JavaScript

And queXF assumes you create the forms yourself (under GPLv2)

for tesseract's license Check here:

Tesseract-OCR is under the Apache license 2.0

I do not think it is problem if somebody promotes his project here (if it is connected to tesseract-ocr or OCR).
It would be better if you share info publicly ;-).

From reading the site it is not open source, but it is free for personal use. I is based on tesseract, and you can find similar open source document management programs on freecode.com if you want.
