I am now sending you the Enrolment Sheet to fill in Names and other details stated in the Form for each student for Year 4 and Year 6 Students. Note that the same Form will be used for both Year 4 and Year 6. Each stream will be listed differently in a different form and do not combine different streams in one form. 

There is another attachment in Word format on Accomodations which is a full detail on the 6th Column of the Enrolment Form on Special Accommodations (this special accommodations is for Students with Special Need). If there are Students with Special Need in your schools please do note this proberly in your Enrolment Sheets. 

The abbreviations and descriptions are as follows:

T: extended Time

P: large Prints 

Q: Quiet Space

B: both extended time and large prints

X: extended time and quiet space

S: quiet space and large print

A: All three accommodations
