The below boxes in the form that unclear, so just follow the suggestion (already asked the immigration officer)

 

 

1. Purpose of visit -  TOURIST

2. Place of residence in respective country -  JUST PUT PHONE NIUMBER

3. Name and address of person to contact in Laos -  JUST LEAVE IT BLANK

4. Proposed address in Laos - JUST PUT YOPUR HOTEL NAME AND IT'S PHONE NUMBER, OR JUST USE THE ONE IN THE SAMPLE.

5. Date - PUT 07/11/2024
