I can't seem to get the following profile to come out of DND mode once it's no longer face down. I need to manually do it each time. How can this be made to work?

Does running the DND Off task that you had before turn it off?
Only other thing I could think of would be to try going through the custom settings and see what does up in there for turning off DND
Add a flash action to the DND Off task to just show text like "DND Off". Just add it to the end of the task you already have for DND Off. Add the exit task back to your profile and uncheck the Restore Settings box for the profile. Turn the phone face down and make sure your profile activates the DND. Once it is active, turn the phone face up and see if you get the flash message.

I'm on a Note 9 with 8.1 so I'm not sure what the difference is.

I have a note 9 also, and I use custom settings for dnd, however it works once only, manually. So I think Samsung is funky with it. I turn it on easily, but off works once. I had it gone with note 8.

I've tried again on my Note 9 and when the display is off the orientation sensor is not firing at all. Even tried the turn on trick but it never fired with the orientation sensor when the display was off.
When I run this as a profile, it shows "Done" but stays in DND. When I run the "Orient Test" task from the task tab, it shows "Done" and successfully gets it out of DND mode(?!)...
The Turn On block time is under Display > Turn On.

Are you sure that you don't have any other profiles running that might turn DND back on or something? Or DND set to certain hours that might interfere? I'm sure you've already checked but just wanted to make sure.
I use the same orientation sensor for a face down vibrate option and it works every time no matter how the accelerometer is set so I'm not sure why it has to be on for the DND to work correctly.
