Probably whatever app handled the first open file is still open at the
first image and just comes to foreground with the second one.

Looking at the code, I can't conceive how this could be a Tasker bug,
and testing it with 2 text files works fine for me if I exit the app
after the first text file is shown.

How do you know that the second Open File call didn't open the correct file?  Are you displaying the loaded image (from the second open file call) using a Tasker action?

(I think I may have run into a similar issue in the past.)

I can confirm this issue, whatever causes it. I set up a voice controlled song picker, and if the music app is in the foreground, it won't work. It's based on Open File. 

That being said, it took me 30 seconds to come up with a fix, just using Kill app, a 150 ms delay and then Open File.
