C++ front-end options

PR862 concerns undocumented options in the C++ front-end.

I'm working on documenting them.

Rather than document -fvtable-gc and -fxref, I intend to move them to
the unsupported_options list on the 3.0 branch, because I do not
believe either of these options works, and I do not think we should be
holding out the implicit promise that they do. Am I incorrect?

I'm going to do the same on the mainline, until someone comes along to
figure out how to make these things work again, or until we decide to
kill them completely.

Speak now, or hold your peace at least until after the release...
