As discussed previously - regarding the need of doing follow up of the mothers and born children, and having data of this follow up so that we can derive logical conclusions about Tapovan output - outcome - impact; pl. find a format attached herewith that has been proposed by TF. Your suggestions and directives would be of immense value in finalizing this important tool.
