Researchers at the National Human Genome Research Institute of the National Institutes of Health (NIH) are conducting an online survey to help them understand how people think and feel about issues related to diabetes. Your response to the survey is completely voluntary.  If you are eligible and choose to complete our survey, you will receive a $10 Amazon gift card as compensation for your participation.

 

To participate:

·         You must be 18 years or older

·         Have been diagnosed with type 1 diabetes mellitus

 

At the bottom of this email you will find a link to our survey. Please read ALL instructions before following the survey link.

 

This survey takes approximately 20-30 minutes to complete, and must be completed in one sitting. Please do not click the link until you are ready to begin your survey. All responses are entirely anonymous.

 

After you have completed this survey, you will be automatically redirected to a second survey, where you will be asked to provide your contact information. This contact information will be used solely for the purposes of providing compensation, and cannot be linked back to your survey responses. Please note: you must provide your contact information in order to receive compensation.

After completing our survey, you will be compensated with a $10 Amazon gift card. Gift card claim codes will be emailed to you at the email address you provide within 10 business days of submitting your survey.
