pkg/aflow/flow/patching: improve and dedup prompts

1. Give few advices on how to phrase commit descriptions.
2. Dedup logic between patching and patch-iteration.
We will have more and more of precious accumulated prompt
remarks over time. It's important to keep the consistent
and shared across both workflows.
