IMPORTANT CHECKLIST
*ONLY 11th graders are eligible to apply for LLSF Scholarships
NEW Deadline date for LLSF Applications is February 20, 2012
(UPLOAD THE FOLLOWING DOCUMENTS)
1. A typed essay (not to exceed 300 words) explaining specific
circumstances that qualify you for the Linda Lorelle Scholarship Fund,
including any challenges you face and/or obstacles you have overcome.
2. A short, typed essay (not to exceed 150 words) describing your
academic aspirations and professional goals.
3. Two recommendation letters, one from a teacher of a core
academic subject: (English, Math, Science, or Social Studies) and
another from an unrelated community member (mentors, teachers, coach,
or clergy) who could specifically discuss your work ethic, experiences
and motivation.
4. Your most recent high school transcript.
5. Copy of a tax return from the person who claims you on their
income tax returns, (custodial parent’s, legal guardian, or other) the
most recent, completed and filed tax return, from 2010 or 2011 include
complete 1040 and copies of wages and tax statements (W2). If parents
are in business, please submit profit and loss statement; if tax
information cannot be provided, please submit financial statement. If
tax information cannot be provided at all, please provide a written
statement explaining the reason(s) that you cannot provide this. If
applicable, provide other documentation, such as: SSI award letter,
Government Assistance award letter, etc…This information will remain
confidential and will be shredded.
Eligibility
Be a current JUNIOR at a school district listed below.
Maintain a grade point average of 2.5 or above on a 4.0 scale
JUNIORS must plan on attending a college or university in the Fall of
2013
